| In case a model to be shipped
to Service Center, the following steps are required: |

information. You will receive a Returns Number for your equipment
after providing these details: |

- A brief description of the problem
you are experiencing with the product

- The name and telephone number of
a person that we can contact regarding the equipment

- Your company name, address, and
telephone number
|
- Ship to information: company name,
address, and telephone number (if not the same as above)

Returns Form |

| Please package your equipment properly
so as not to incur any damage |
| Include the Hamlet Returns Form |
| We also recommend insuring your equipment/shipment
to Hamlet for loss or damage |
